When I have this plugged into my phone playing music its great until I turn the volume up all the way and then on loud parts of the song it just makes terrible noise. Is this because of the power limit on this chip? Could someone explain??

Could this amp be used as a headphone amp if I used a preamp with a volume control?
I don't see why not, although this will have about 10 times the power output of a standard headphone amplifier. You will need to be VERY careful with volume levels.

I would like to build a complete amp from this item. What are all the parts that I need to complete it? Case enclosure, volume knob, speaker and rca connectors, etc? What type of wire(s) do you use to solder to the board? Power supply too?

No, this amplifier is not bridgeable.

I'm thinking of a project using this board and I may want to throw a 4 ohm speaker on one channel and an 8 ohm on the other. Is that a terrible idea, or will that work out?
There is nothing wrong with doing this on this amplifier.

Are the J1,J2 sockets for input (same as the 3.5mm jack on PCB)? If so, which is L/R? Same with Output, which channel is Ch 1/2 (L,R)? I'm planning on using sig input directly from a USB bluetooth receiver, powered by a step-down DC regulator (12v to 5v).
Yes, J1 and J2 are the inputs. J1 is Left and J2 is Right. J3 is Left output and J4 is Right output.
