could i just use the high level inputs only? cant use high level out. satellites wires are in wall which are 4 ohms each speaker. if using the high level inputs only, will the amp still see a 4 ohm load?
Yes you can use only the high level inputs. The input type does not effect what impedance the amplifier will see.

Is this the right amp for the set of 4 puck tactile tranducers, Part # 300-389? If so, will just one amp be enough, or will I need another one?
This amp will power them, but in order to use them to their full potential I would recommend the SA70 instead.

No. This is a 25 watt sub amp. You need either the SPA500 or SPA1000

Building Voxel subs, system uses Lepai which powers small TV soundbar. What would be best option for input to the plate amp? Split the RCA outs from the TV (one to Lepai, the other to plate amp RCA in) or connect using speaker outs from Lepai?
In order to control the subwoofer volume level with the Lepai amplifier you will want to use the speaker level inputs and outputs.

Does this need to be a certain distance from the sub magnet? Also, should the plate be warm even after gain set to off (light off) but plugged-in all night? Thanks.
There is no minimum distance from the subwoofer magnet, it will not effect the amplifier. It is normal for the plate to be warm even when turned off.
